Identify clients needing case management, focusing on crisis stabilization and facilitating linkages to community service supports and medical providers. Provide outreach and individualized support to clients based on provider recommendations and support needs identified by the client. Provide assistance in clarifying transition needs, gaps in services and supports post inpatient hospitalization & crisis hotline operator.

Direct patient care in conjunction with Registered Nurses and Physicians in Emergency Department setting. Specimen collection, labeling and transfer to hospital laboratory via pneumatic system. Procedural assistance, support with activities of daily living (ADL’s) including but not limited to eating, dressing and toileting. Primary focus in support of patients under mental health duress, intake and assessment into psychiatric observation unit of Emergency Department

Supports the planning, implementation and delivery of health education topics for patients, provides individual and group education and participates in community events and outreach activities. Offers preventative health care such as confidential screening for people who are at high risk for HIV, STI’s. Creation of culturally and linguistically appropriate curriculums, program planning, and retention services for patients living with HIV/AIDS (PLWHA).
